Improving Health of the Uninsured with a Blended Physical & Behavioral Health Model

Mountain Hope Good Shepherd Clinic plans to evaluate the implementation of a blended care model to increase early screening and identification of behavioral health needs, and healthy behaviors of our uninsured patient population. The blended care model will impact patients of all ages and ethnicity who live or work in Sevier County. The current model includes medical, dental, and limited behavioral medication management. The proposed blended model of care will include screening for depression, anxiety, and other mental health conditions, and early intervention treatment with a licensed behavioral therapist embedded in the primary care team. The team will include a health coach to engage patients in behavior changes to improve their future health.
